dc.description.abstractRESUMO - Introdução: Os sistemas de saúde enfrentam desafios significativos de sustentabilidade, condicionados por recursos financeiros limitados e pela crescente procura de cuidados. A necessidade de garantir eficiência, eficácia e custo-efetividade, mantendo o utente no centro dos cuidados, torna-se cada vez mais premente. A metodologia Lean e o Lean Thinking na área da Saúde, visam eliminar desperdícios e criar valor para o utente. Objetivos: Analisar os principais desafios no circuito do doente para cirurgia eletiva, explorar o potencial do Lean Thinking através do Value Stream Mapping como ferramenta de melhoria contínua no contexto de um serviço público de saúde. Métodos: Estudo transversal com quatro etapas: (1) diagnóstico do percurso do utente em cirurgia eletiva, através do Value Stream Mapping; (2) recolha de tempos; (3) análise dos tempos; e (4) desenho de soluções orientadas para a otimização do circuito e criação de valor para o utente. Resultados: Da aplicação do Lean Thinking ao percurso do doente cirúrgico eletivo verificou-se maior desperdício na enfermagem pré-operatória e na demora pré-operatória. O VSM evidenciou um funcionamento em sistema empurrado, identificou demoras no acolhimento, esperas e riscos desnecessários no bloco operatório, e apontou para a utilização de soluções como o Kanban e o 5S. Conclusões: O Lean Thinking tem demonstrado ser eficaz na redução de tempos de espera e internamento, aumentando a eficiência e a qualidade dos cuidados de saúde. A análise do percurso cirúrgico em Neurocirurgia evidenciou oportunidades de melhoria, reforçando o Lean como um processo contínuo e evolutivo para otimizar o acesso à saúde.por
dc.description.abstractABSTRACT - Introduction: Health systems face significant sustainability challenges, constrained by limited financial resources and the growing demand for care. The need to ensure efficiency, effectiveness, and cost-effectiveness, while keeping the patient at the center of care, is becoming increasingly urgent. The Lean methodology and Lean Thinking in healthcare aim to eliminate waste and create value for the patient. Objectives: To analyze the main challenges in the surgical pathway of patients undergoing elective surgery, and to explore the potential of Lean Thinking through Value Stream Mapping as a tool for continuous improvement in the context of a public healthcare service. Methods: Cross-sectional study with four stages: (1) diagnosis of the patient pathway in elective surgery through Value Stream Mapping; (2) time collection; (3) time analysis; and (4) design of solutions aimed at optimizing the pathway and creating value for the patient. Results: Applying Lean Thinking to the elective surgical patient pathway revealed greater waste in preoperative nursing and preoperative delay. The VSM highlighted a push-system functioning, identified delays in admission, waiting times, and unnecessary risks in the operating room, and pointed to the use of solutions such as Kanban and 5S. Conclusions: Lean Thinking has proven effective in reducing waiting and hospitalization times, increasing efficiency and the quality of healthcare. The analysis of the surgical pathway in Neurosurgery highlighted opportunities for improvement, reinforcing Lean as a continuous and evolving process to optimize access to healthcare.eng
