CONTEXTO: O Farmacêutico Comunitário português tem conseguido lidar com a mudança e com os novos desafios do sistema de saúde. Em 2007, nova legislação deu ao Farmacêutico Comunitário a possibilidade de prestar novos serviços, para além da dispensa de medicamentos. Juntamente com a actual tendência dos cuidados de saúde centrados no doente, com a inovação tecnológica e com a reforma dos cuidados de saúde primários, surge agora uma oportunidade para aperfeiçoar o papel que o Farmacêutico Comunitário pode ter no sistema de saúde português, sendo objectivo deste trabalho analisar as possibilidades de participação do futuro Farmacêutico Comunitário no Sistema de Saúde em Portugal.
MÉTODOS: Para a realização deste estudo, usámos o método de análise de cenários, visando definir cenários para o futuro dos Farmacêuticos Comunitários em Portugal. Convidámos profissionais conhecedores do contexto para a realização de 2 workshops de cenarização. Após identificarmos as principais incertezas que determinarão o futuro do Farmacêutico Comunitário, construímos 3 cenários plausíveis representando, cada um deles, 3 futuros alternativos.
RESULTADOS: A informação recolhida no primeiro workshop permitiu-nos identificar as incertezas críticas que irão influenciar o futuro do Farmacêutico Comunitário. Os eixos que parecem ser os que mais determinarão o futuro são: a Capacidade de Desenvolvimento de Serviços, reflectindo as diferenças em inovação e desenvolvimento de serviços entre farmácias; e o Ambiente Legislativo, reflectindo a maior ou menor tendência para a liberalização do mercado das farmácias e do próprio sistema de saúde. Esta análise permitiu-nos enquadrar o futuro do Farmacêutico Comunitário em três cenários que denominámos “Pharmacy-Mall”, “e-Pharmacist”, e “Reorganize or die”.
CONCLUSÃO: No cenário “Pharmacy-Mall”, o papel do Farmacêutico Comunitário será o de supervisionar a dispensa de medicamentos na farmácia, podendo ter severas implicações no emprego dos Farmacêuticos Comunitários. No cenário “e-Pharmacist”, o Farmacêutico Comunitário estará mais dedicado à prestação de serviços para o sistema de saúde, podendo exercer fora do contexto da farmácia. No cenário “Reorganize or die”, o Farmacêutico Comunitário será levado a desenvolver e vender novos serviços para assim aumentar os lucros da farmácia. Independentemente de qualquer dos cenários pode-se concluir que o Farmacêutico Comunitário terá de obter novas competências em áreas que tradicionalmente não são do âmbito do farmacêutico tais como, técnicas de gestão, domínio das tecnologias de informação e técnicas de marketing.
INTRODUCTION: The Portuguese Community Pharmacists are facing the normal challenges of an evolving health system. Recently, new legislation enabled Community Pharmacists to provide new services, beyond the traditional role of dispensing. Considering the current trend of patient centered healthcare, the primary care reform and the technological evolution, the objectives of this study are to analyze the future Community Pharmacists’ participation in the Portuguese health system and what will be the education needs of these professionals. METHODS: We used the scenario analysis’ method, aiming to define scenarios for the future of the Portuguese Community Pharmacist. We invited 8 personalities, to 2 scenario workshops, in December 2010 and January 2011. After identifying the critical uncertainties we designed 3 plausible scenarios, representing 3 alternative futures RESULTS: The workshops allowed us to identify the critical uncertainties that will be the drivers of change for the Community Pharmacists. These were found to be the Ability to develop Services, with more or less innovation between pharmacies, and the Legislative Environment, with a more or less liberalized pharmacy market and health system. This analysis enabled us to frame the future of the Community Pharmacists in 3 distinct scenarios, which we named “Pharmacy-Mall”, “e-Pharmacist”, and “Reorganize or die”. CONCLUSIONS: In the “Pharmacy-Mall” scenario, the main role of the Community Pharmacist will be supervising dispensing procedures. The Community Pharmacists will then need more managing skills and knowledge of administrative matters. In the “e-Pharmacist” scenario, the Community Pharmacists will be dedicated to providing services beyond dispensing, thus needing abilities in areas such as disease management and IT skills. In the “Reorganize or die” scenario, the Community Pharmacist will develop new services and innovate ways to provide them, needing sales techniques and customer relations abilities. Regardless of any scenario, it seems clear that the future Community Pharmacists will have to obtain abilities that traditionally are outside their competences, namely researcher, managing, customer relations and IT skills.
