Exploring the Cellular Basis of Autoimmunity using single-cell Transcriptomic Analysis of lmmune Cells from Antiphospholipid Syndrome Patients

Antiphospholipid Syndrome (APS) is a rare autoimmune disease, characterized by the presence of antiphospholipid autoantibodies (aPL) in circulation, leading to recurrent thrombotic events. Alt hough aPL levels can fluctuate over time, some patients become seronegative. Since patients are treated exclusively with anticoagulant drugs, the absence of immunomodulatory treatment makes APS the ideal model to study immune dysregulation. Preliminary data demonstrated an increase in the frequency of T follicular regulatory and acti vated T follicular helper cells, accompanied by a decrease in switched memory B cells, in seropositive patients (aPL+) compared with healthy (HD) and seronegative donors (aPL-). It was hypothesized that, during the course of APS, there are transcriptional changes in T and B cell subsets that contribute to aPL fluctuation and disease exacerbation. Therefore, the transcriptomes of circulating CD4+ T and B cell subsets were analyzed using single-cell RNAseq. B cell differentiation was associated with meta bolic alterations, with unswitched memory B cells revealing a distinct metabolic profile across APS conditions. These results revealed divergences during disease progression, with signatures shared by aPL+ and aPL- patients but also signatures that distinguish the two groups. Altogether, this approach provides an innovative way of identifying key mechanisms of autoimmunity with single-cell resolution.
A Síndrome Antifosfolipídica (SAF) é uma doença autoimune rara, caracterizada pela presença de anticorpos antifosfolipídicos (aPL) em circulação e associada a eventos trombóticos recorrentes. Apesar dos níveis de aPL variarem ao longo do tempo, alguns doentes tornam-se seronegativos. Como o tratamento se baseia exclusivamente em anticoagulantes, a ausência de imunomodulação torna SAF no modelo ideal para estudar a desregulação imune. Dados preliminares demonstraram um aumento das células T foliculares reguladoras e T foliculares auxiliares ativadas, acompanhado por uma diminuição das células B de memória pós-troca de isotipo em doentes seropositivos (aPL+), comparativamente a dadores saudáveis (HD) e seronegativos (aPL-). Foi hipotetizado que ocorrem alterações transcripcionais nas células T e B que contribuem para a flutuação dos aPL e a exacerbação da doença. Assim, os transcriptomas das células B e T CD4+ foram analisados usando sequenciação de RNA com resolução celular (single-cell RNAseq). A diferenciação das células B foi associada a alterações metabólicas, onde as células de memória sem troca de isotipo apresentaram perfis metabólicos distintos entre condições de SAF. Estes resultados revelaram divergências na progressão da doença, com assinaturas comuns entre doentes aPL+ e aPL-, mas também assinaturas distintas. Esta abordagem inovadora permite identificar mecanismos chave da autoimunidade, com resolução celular.
