RESUMO - Em princípio, é possível promover-se a eficiência produtiva
dos hospitais públicos criando incentivos que levem os diferentes
prestadores de cuidados destas instituições a minimizar
os custos e a manter, simultaneamente, um determinado
padrão de qualidade. De entre os instrumentos
disponíveis, a utilização de sistemas de financiamento prospectivo
assegura estes incentivos.
Neste contexto, o principal objectivo deste estudo é o de
tentar analisar se a produção hospitalar se realizou de
forma eficiente durante um período de tempo em que os
hospitais públicos foram financiados por orçamentos e
incentivados a reduzir a demora média por doente tratado
em cada um dos grupos de diagnóstico homogéneo.
Para testarmos a estrutura produtiva dos hospitais estimamos
uma função de custos totais translogarítmica, usando
uma amostra constituída por 36 hospitais distritais para o
período de onze anos (1984-1994).
Concluímos que o hospital distrital (com uma dimensão
média de 241 camas) tem potencialidades para produzir de
forma eficiente, na medida em que os dados nos sugerem
que explora economias de escala globais e, na maioria das
especialidades, economias de produção conjunta. Os resultados
indicam-nos também a existência de uma quebra nos
custos, em termos reais, de 1984 a 1994 e ainda uma
grande dispersão nos custos dos diferentes hospitais.
ABSTRACT - In principle, it is possible to promote productive efficiency in public hospitals by giving incentives to its providers so that they minimise costs, while maintaining the quality of care. Prospective financing systems are commonly believed to assure these objectives. Therefore, the main aim of this paper is to analyse whether public hospitals have produced efficiently over a period in which they were funded by budgets and given incentives to decrease length of stay in each diagnostic related group. In order to test hospital productive structure a translog cost function was estimated, using a sample of 36 district hospitals for a period of 11 years (1984-1994). We have concluded that the district hospital (with an average size of 241 beds) can produce efficiently, since data suggest that it is exploring overall scale economies and, in most specialities, economies of scope. The results also suggest that total costs have been decreasing, in real terms, over time and that there is a wide spread in costs among hospitals.
