RESUMO - Os constrangimentos originados pela actual estrutura de
oferta de cuidados de saúde e os novos desafios colocados
aos sistemas de saúde provocados pela evolução constante
e cada vez mais rápida do seu ambiente externo exigem
uma resposta efectiva por parte dos responsáveis pela gestão
desses mesmos sistemas. A integração de cuidados de
saúde pode assim constituir uma resposta organizacional
aos novos paradigmas dos sistemas de saúde. Apesar de
não se tratar de uma opção estratégica consensual nem
permitir uma operacionalização linear, as suas potencialidades
são amplamente reconhecidas, sobretudo na sua
dimensão estrutural vertical, que permite uma focalização
na criação de valor para o utente e necessariamente na
concentração do desenvolvimento das actividades nas fases
primárias da doença. A integração vertical de cuidados
atinge a sua plenitude nas dimensões funcionais relacionadas
com as actividades clínica, de informação e financeira.
Com o presente estudo pretende-se rever os principais
aspectos teóricos da integração vertical, apresentar as suas
principais barreiras e condições facilitadoras.
ABSTRACT - The new challenges motivated by the constant and faster evolution of environmental context demand an effective answer by health care managers. The movement towards integration of healthcare provision may constitute the organizational answer that allows facing those new and systemic challenges to health care systems around the world. Although there is no international consensus about its potential, mainly on vertical integration, this reform can create value for the client and straightforward strategic focus on primary steps of the continuum of care. The vertical integration reaches its plenitude on functional dimensions, related particularly with clinical, information and financial issues. This paper provides a theoretical review of the principal aspects of vertical integration movement, its facilitators and principal barriers.
