O artigo analisa as particularidades dos sistemas de saúde de Pequenos Estados Insulares em Desenvolvimento, também designados como países insulares, destacando o seu isolamento geográfico, a fragmentação territorial e vulnerabilidade climática, como condicionantes das especificidades dos sistemas de saúde. O texto identifica os principais desafios dos sistemas de saúde insulares: formação e fixação de recursos humanos especializados em localizações remotas, escassez de recursos financeiros que limitam o acesso a equipamentos médicos e medicamentos, elevado número de evacuações de utentes e edificação e manutenção de infra-estruturas de saúde e de tecnologias médicas em territórios geográficos pequenos e isolados. Finalmente, soluções implementadas pelos países lusófonos insulares para fazer face às dificuldades de acesso aos cuidados de saúde impostas pela insularidade são apresentadas, destacando a telemedicina e a contratação de recursos humanos especializados como estratégias para atingir a cobertura universal de saúde. The article analyzes the particularities of health systems of Small Island Developing States, also known as island countries, highlighting their geographical isolation, territorial fragmentation and climate vulnerability as determinants of the specificities of their health systems. The text identifies key challenges of insular health systems: training and retention of specialized human resources in remote locations, scarce financial resources that limit access to medical equipment and medicines, high number of medical travels, and construction and maintenance of medical infrastructures and technologies in small and isolated territories. Finally, solutions implemented by the Portuguese-speaking countries to address the difficulties of access to health care imposed by insularity are presented, highlighting telemedicine and hiring of specialized human resources as potential strategies to achieve Universal Health Coverage.
