RESUMO - Partindo de uma perspectiva histórica sobre o desenvolvimento
dos discursos e práticas relativas à desviância, à
doença e à saúde, em particular a mental e a pública, identificam-
se dois modelos de intervenção — o modelo clínico
e o comunitário. Seguidamente, apontam-se limitações e
convergências entre cada um desses modelos, englobando-
-os no âmbito da segunda revolução da saúde e do nascimento
da moderna saúde pública, a qual se centrou sobre
os conceitos de prevenção da doença e promoção da saúde.
Paralelamente, questiona-se um conjunto de mecanismos
no domínio da saúde, os quais são entendidos enquanto
dispositivos ao serviço de ideologias. A este propósito é
realçada a necessidade de contextualizar a discussão no
âmbito mais vasto da política e de uma economia de mercado
que, cada vez mais, procura legitimar-se a partir de
formas aperfeiçoadas e subtis de conhecimento do destinatário
sobre o qual exerce o seu poder — o sujeito psicológico.
ABSTRACT - Starting from a historical perspective about the development of deviance, disease, and health discourses and practices, particularly the mental and public, two intervention models are identified — the clinical model and the community model. The limitations and convergences between these models are pointed out and conclobated in the scope of the second health revolution and of the rise of modern public health, which is centered on the concepts of disease prevention and health promotion. Parallelly, we discuss a set of mechanisms in the domain of health, which are understood as ideological devices. With this purpose the necessity to address the discussion to the broader context of politics and market economy is enhanced since, more and more, this lost issue tries to pursuit its legitimacy from improved and subtle forms of knowing the addressee, above whom the power is exerted — the psychological subject.
