RESUMO - Introdução: As características das infraestruturas hospitalares estão diretamente
associadas com a qualidade dos cuidados prestados. Resultam de investimentos
volumosos e são de carácter relativamente permanente, pelo que o seu planeamento
ou reformulação deve ser baseado na melhor evidência. A macrotendência existente
para a ambulatorização e o aumento da incidência do cancro amplifica o peso dos
Hospitais de Dia Oncológicos no futuro das instituições de saúde, criando desafios
acrescidos às infraestruturas que suportam a sua atividade.
Objetivos: Realizar uma proposta de reformulação de um Hospital de Dia Oncológico
em S. Miguel, nos Açores, com base nas recomendações resultantes de um estudo de
caso.
Metodologia: Foi desenvolvido um estudo de caso de dois Hospitais de Dia
Oncológicos. Foi levantado o layout de cada unidade. Recorreu-se a dados sobre
recursos humanos, volume e tipo de atividade. Realizou-se uma observação estruturada
com recurso a um guião pré elaborado.
Resultados: A proposta baseou-se nas seguintes recomendações: criação de uma
nova zona de espera; reconfiguração da zona de cadeirões e da zona de camas;
delimitação de áreas; definição de uma bancada de preparação; descentralização dos
postos de enfermagem; requalificação do pátio; reestruturação dos arrumos.
Conclusões: A figura do administrador hospitalar tem a capacidade de ser o mediador
entre as diversas partes que integram o planeamento ou a reformulação das
infraestruturas hospitalares. A avaliação e o registo de como estas respondem aos
requisitos impostos pela atividade é uma forma de impedir a perpetuação de erros já
cometidos.
ABSTRACT - Introduction: The characteristics of hospital infrastructures are directly associated with the quality of care. They result from substanctial investments and are relatively permanent, so their planning or redesign should be based on the best evidence. The existing macrotrend towards outpatient care and the increasing incidence of cancer amplifies the importance of Cancer Day Hospitals in the future of healthcare institutions, creating additional challenges for the infrastructures that support its activity. Objectives: To propose a redesign of a Cancer Day Hospital in São Miguel, Azores, based on recommendations from a case study. Methodology: A case study of two Cancer Day Hospitals was conducted. The layout of each unit was surveyed. Data on human resources, volume and type of activity were collected. Structured observations were carried out using a pre-prepared guide. Results: The proposal was based on the following recommendations: creation of a new waiting area: reconfiguration of chairs and beds area; delimitation of area; definition of a preparation counter; decentralization of nursing stations; refurbishment of the patio; restructuring of storage areas. Conclusions: The hospital administrator has the ability to mediate between the various parties involved in the planning or redesign of hospital infrastructures. Evaluating and registering how these infrastructures respond to the requirements imposed by the activity is a way to prevent the perpetuation of past mistakes.
