RESUMO - O autor descreve sucintamente os vários níveis de prevenção
em saúde, incluindo o mais recente: a prevenção quaternária.
Este quinto nível de prevenção visa detectar indivíduos
em risco de sobretratamento (overmedicalisation),
por forma a protegê-los do intervencionismo médico inapropriado,
além de visar capacitar os utentes enquanto
consumidores de cuidados de saúde.
A pertinência da prevenção quaternária resulta dos custos
crescentes em saúde nos últimos quarenta anos, decorrentes
quer do envelhecimento da população, quer da inovação
tecnológica hospitalar.
O autor aborda ainda o «marketing do medo» (marketing of
fear), que consiste na sobrestimação de situações patológicas
na população em geral, bem como na «medicalização»
de factores de risco, visando alargar o mercado dos consumidores
de saúde e, consequentemente, o mercado farmacêutico.
É salientada a importância da análise das decisões clínicas
como forma de aumentar a qualidade do acto médico e a
utilização eficiente dos métodos diagnósticos e terapêuticos.
ABSTRACT - The author describes briefly the several identified levels of prevention including the most recent one: the quaternary prevention. This fifth level of prevention aims to identify patients at risk of «overmedicalisation» — in order to protect them from new medical invasion — and to empower patients/health consumers. Quaternary prevention is pertinent because of the continuing growth of health care expenditure during the last 40 years, secondary to ageing of the population and to hospitals’ technologic innovation. The author describes the so-called «marketing of fear», which consists on the over-estimation of disease and medicalisation of risk factors in order to expand markets for new products. The importance of clinical decision analysis is emphasised, as a way to improve medical practice and to promote an efficient use of diagnostic and therapeutic tools.
